OVERVIEW

Roles and Responsibilities
Internship will not work unless all people involved are aware of their personal roles and responsibilities. Take a moment to familiarize yourself with these processes in order to avoid confusion in the future.

PRE-INTERNSHIP

Intake Form
Please fill out and submit an Intake Form when you are ready to start thinking your about your Internship experience. Once the Internship Program Office receives this from you, we will call you to set up an orientation session.


Training Agreement/Learning Objectives
Once you secure an internship site and have a faculty mentor, you will need to obtain original signatures on the Training Agreement. Also, be sure to work with your Site Supervisor and Faculty Mentor in order to determine your structured Learning Objectives. Once this is submitted to the Internship Office, we will give you a registration code for your internship.


DURING THE INTERNSHIP

Timesheet
Throughout your internship, you are responsible for tracking your hours of experience. You MUST have your employer verify your hours and sign the Timesheet before you return the original copy to the Internship Program Office. If you have multiple timesheets, you can either submit them as they are completed OR at the end of your internship experience.

POST INTERNSHIP

Student Self-Evaluation
After you finish your internship, you are required to complete a Self-Evaluation. This evaluation allows you to reflect on your experience as well as your internship site. The Self-Evaluation must be completed and returned to the Internship Program Office before you will receive a final grade.
